Project for Assistance in Transition from Homelessness helps people who are homeless and who have a mental health challenge to move forward and leave homelessness behind them. HOPES’ PATH Service Facilitator works hand in hand with PATH Participants to help them access mental health services, income, housing, and other critical services that will help make their journey out of homelessness into a more stable and fulfilling life become reality.

Support and Connections

The winter of 2018-2019 was harsh. Over the last three years, Stacy has spent her nights between staying with friends and sleeping in her vehicle. Eventually, her van became her primary home. Looking for a positive change, Stacy reached out for help.

Connecting with the street outreach team, she would meet them with a big smile and optimistic outlook as they as they approached her van at night. The team provided her with hot coffee, blankets, hand warmers, food and water, and even some gas cards to keep the van running and warm during periods of extreme cold.

Beyond outreach, Stacy started meeting with Kathie (former PATH Service facilitator). They searched for housing and got Stacy connected to community mental health services. They submitted a lot of housing applications with few results, until Stacy was accepted into a Permanent Supportive Housing project through the Racine Continuum of Care’s Coordinated Entry system. In spite of the challenges of the COVID-19 pandemic, she settled into her own apartment in May 2020, ended her homelessness, and continued with the great progress she has been making in her life.
